Support Includes:

Eligibility Criteria:
Individuals of all ages needing temporary support due to caregiver absence or relief
Must have documentation of need (medical, behavioral, or functional)
Eligible through:
APD Waiver Program (with support coordinator referral), Private Pay (no referral needed), Long-Term Care Insurance or other qualified providers
